PMMC’s Hospice Program is designed to meet the physical, emotional and spiritual needs of patients and families who are dealing with terminal illness. This care is provided in a patient’s home, assisted living facility, skilled nursing facility, or the hospital. We strive for excellence in the delivery of our services.

What is hospice care?
Hospice care is provided to individuals who have a terminal diagnosis, have chosen end of life care, and have readily available caregiver support.
Who is the hospice team?
Hospice care is provided to patients and their families by an interdisciplinary team including: Hospice medical director, personal physician, registered nurses, licensed practical nurses, certified nursing assistants, medical social worker, chaplain, nutritional support and hospice volunteers. Our hospice team is on-call 24 hours a day, 7 days a week.
What is the focus of hospice?
Our focus is on comfort care at the end of life. Our hospice team offers symptom management through medications prescribed by your physician and tailored to your needs. Alternative therapies for managing symptoms may include aroma therapy, massage therapy, reiki, pet therapy and music therapy.
When does hospice care end?
Our hospice team offers a comprehensive bereavement program for the family after their loved one has passed away. The 13 month program is designed for the needs of the family and may include visits from the hospice team, monthly support group meetings, letters, phone calls from a hospice bereavement volunteer, a grief newsletter, and a memorial service.
Who pays for hospice?
Medicare-eligible individuals can receive hospice care through the Medicare hospice benefit. This includes services from the hospice team, durable medical equipment, and medications related to the terminal illness or for comfort.
Managed care and other insurance companies have individualized hospice benefits. The hospice team can assist with determining your personal benefits.
