PMMC's Home Care is designed to meet the individual needs of the patient. This care is provided in the patient's home by qualified, professional employees. We strive for excellence in the delivery of our services.

What is Home Care?
Home Care is intermittent skilled care provided in the home at scheduled intervals.
What is skilled care?
Skilled Care Services provided in the home may include:
- Skilled Nursing - Registered nurses assess a patient's needs and provide intermittent nursing visits for the treatment of illness or injury, often training family/caregivers to assist in treatment. Home Care at PMMC provides Primary Care Nursing.
- Home Infusion Therapy - For diversified treatments of illness or injury.
- Enterostomal Therapy - To provide treatment for all types of wounds and ostomies.
- Physical Therapy - To maximize a patient's level of functioning.
- Occupational Therapy - To improve a patient's independence with daily living activities.
- Speech Therapy - Help patients overcome or adapt to changes in speech, language or swallowing disorders.
- Home Health Aide - Professionally supervised aides provide assistance with personal care needs.
- Medical Social Worker - Provides assistance with locating and accessing community resources.
How can I receive Home Care?
If Home Care is required, please call the Home Care department and we will then contact your physician for approval to start services and, if necessary, obtain authorization from your insurance company.
What are the costs to me?
Most insurance pays for doctor ordered, medically necessary and approved services.
PMMC's Home Care is Medicare certified and JCAHO accredited.
